Job Description

Delivering compassionate care to individuals and their families during lifes most delicate moments defines this vital field-based nursing opportunity. As a Registered Nurse specializing in hospice, you will provide skilled, patient-centered end-of-life care in homes, assisted living, or nursing facilities, ensuring each individuals comfort and dignity.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation as needed

Position details include a full-time, 13-week contract with a manageable caseload averaging 45 patient visits daily. Utilize your expertise in Home Care Home Base (HCHB) while traveling within the designated local area (with mileage reimbursement provided).

Key Responsibilities:

  • Conduct thorough nursing assessments and develop individualized hospice care plans.
  • Deliver skilled nursing interventions focused on symptom management and overall comfort.
  • Administer medications and manage pain per hospice protocols.
  • Monitor patient status, promptly updating care teams and physicians.
  • Educate patients and families on comfort measures, symptom progression, and emotional support.
  • Work collaboratively with social workers, chaplains, aides, and volunteers.
  • Complete timely, accurate documentation in HCHB.
  • Participate in team meetings and quality improvement activities.
  • Ensure adherence to hospice policies and regulatory guidelines.

Qualifications:

  • Active RN license in good standing.
  • At least one year of recent hospice experience.
  • Proficiency in Home Care Home Base (HCHB).
  • Strong clinical assessment, communication, and organizational skills.
  • Understanding of pain management, end-of-life care, and family support.
  • Reliable transportation, valid drivers license, and insurance.
  • Compassionate, emotionally resilient approach.
  • (Preferred) BSN; prior experience in palliative care; knowledge of Medicare hospice regulations.
