Hospice Registered Nurse (RN) Clovis, CA
A clinical role focused on delivering compassionate hospice care to patients facing terminal illnesses within their homes or residential settings. This full-time contract spans 13 weeks and requires an empathetic nurse skilled in symptom management, communication, and interdisciplinary collaboration.
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized hospice care plans.
- Provide skilled nursing care emphasizing pain control and patient comfort.
- Administer medications and treatments per physician orders and hospice protocols.
- Monitor and promptly report changes in patient condition to the care team.
- Educate patients and families about disease progression, comfort measures, and medication.
- Collaborate with social workers, chaplains, aides, and volunteers to support patients physical, emotional, and spiritual needs.
- Document accurately and timely using the Home Care Home Base (HCHB) system.
- Participate in case conferences, team meetings, and quality improvement efforts.
- Ensure adherence to hospice policies and regulatory guidelines.
Qualifications:
- Active RN license in good standing within the state.
- Minimum one year of recent hospice nursing experience.
- Proficient in Home Care Home Base (HCHB).
- Strong clinical assessment and communication skills.
- Knowledge of pain management and end-of-life care principles.
- Reliable transportation with a valid drivers license and insurance.
- Ability to work independently in a field-based setting.
- Compassionate and emotionally resilient approach.
Preferred:
- BSN degree.
- Experience in palliative or end-of-life care.
- Familiarity with Medicare hospice regulations and documentation.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM with occasional weekend or on-call rotation.
- Caseload averages 45 patient visits daily.
- Field-based work includes patient homes, assisted living, and nursing homes with mileage reimbursement.
