Seeking a skilled and compassionate Registered Nurse with home health experience to support patients in a field-based role. As a Home Health RN, you will deliver patient-centered care directly in patients homes, ensuring continuity, quality, and collaboration with the broader healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and promptly communicate updates to physicians and the care team.
• Educate patients and families about disease management, medications, and preventive health measures.
• Accurately document all care provided in Home Care Home Base (HCHB).
• Collaborate with interdisciplinary professionals such as therapists, social workers, and home health aides.
• Attend case conferences, staff meetings, and engage in continuing education.
• Adhere to all regulatory and professional standards for documentation and patient care.

Requirements:

• Active RN license (in good standing) in the state of practice.
• At least one year of recent home health nursing experience (field-based strongly preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS is essential.
• Strong assessment, organizational, and communication skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to maintain a flexible schedule and work independently.

Preferred Qualifications:

• Bachelors degree in Nursing (BSN).
• Experience managing 25-32 points per week.
• Familiarity with Medicare and home health compliance.
