Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop tailored care plans.
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate patient progress and communicate changes to the care team and physicians promptly.
• Provide education and support to patients and families regarding disease processes, medications, treatment plans, and preventive care.
• Document all care accurately and in real time using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate care.
• Participate in case conferences and staff meetings, and engage in ongoing professional development.
• Ensure adherence to all agency, state, and federal documentation and care standards.

Qualifications:

• Active RN license (in good standing).
• At least 1 year of recent Home Health nursing experience, with preference for field roles.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong assessment, organizational, and communication skills.
• Ability to manage a caseload of 2532 points per week (35 patient visits daily).
• Reliable vehicle, valid drivers license, and active auto insurance.
• BSN preferred; experience with Medicare regulations a plus.

$1,839 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
