Schedule and Assignment:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based (travel to patient homes with mileage reimbursement)

Key Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing services: wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, identify changes in condition, and collaborate promptly with the care team and physicians
• Educate patients and families on treatment plans, medications, and preventive health strategies
• Maintain precise and timely documentation using Home Care Home Base (HCHB)
• Work closely with physical therapists, occupational therapists, social workers, and home health aides for coordinated care
• Participate in case conferences, staff meetings, and ongoing professional development
• Ensure adherence to all regulatory and agency protocols

Qualifications:

• Active, unrestricted RN license in state of practice
• At least 1 year recent Home Health nursing experience (field work preferred)
• Proficiency with HCHB and demonstrated OASIS experience
• Strong clinical assessment, communication, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage a flexible schedule

Preferred:

• BSN
• Experience managing 2532 points/week
• Familiarity with Medicare and home health compliance
