Experienced and compassionate nurses passionate about delivering individualized patient care thrive in home health roles. This position focuses on providing skilled nursing services directly in patient homes, ensuring personalized, high-quality, and patient-centered outcomes. Working independently, you will oversee all aspects of care, from assessments and planning to education and coordination with interdisciplinary teams.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Your role will include traveling daily within a defined territory, with a caseload expectation of 2532 points per week (typically 35 patient visits each day). Accurate documentation using Home Care Home Base (HCHB) and OASIS assessments is vital to meeting care goals and complying with regulations.
Responsibilities:
- Perform comprehensive OASIS assessments and develop tailored care plans
- Deliver skilled nursing services including wound care, IV therapy, medication administration, and managing chronic conditions
- Monitor patient progress, identify condition changes, and communicate promptly with care teams and physicians
- Educate patients and families about disease management, medications, and preventive care
- Maintain accurate, timely documentation in HCHB
- Coordinate with physical therapists, occupational therapists, social workers, and home health aides
- Participate in case conferences, staff meetings, and ongoing education
- Comply with all agency and regulatory requirements
Qualifications:
- Active RN license in good standing (state of practice)
- Minimum 1 year of recent home health (field) experience
- Proficiency with HCHB and OASIS required
- Strong critical thinking, assessment, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN preferred; knowledge of Medicare/home health compliance highly valued
