An opportunity is available for a skilled Home Health Registered Nurse (RN) to provide patient-centered nursing care in home settings across Clayton, NC. This full-time contract role requires delivering comprehensive nursing assessments, creating individualized care plans, and managing ongoing patient needs with a focus on clinical excellence and effective communication.

Key Responsibilities:

• Conduct detailed OASIS assessments and develop patient-specific care plans.
• Administer skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and communicate changes promptly to the interdisciplinary care team and physicians.
• Educate patients and their families on treatment plans, medications, and preventive health practices.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to deliver coordinated care.
• Participate in case conferences, staff meetings, and continuing education.
• Ensure compliance with regulatory and agency standards in all aspects of care.

Qualifications:

• Active RN license in good standing within the state of practice.
• Minimum of 1 year of recent home health experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment abilities and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with excellent communication, organizational, and time management skills.
• BSN preferred.
• Experience managing a caseload of 2532 points per week is a plus.
• Familiarity with Medicare and home health compliance standards is an advantage.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• Field-based with daily travel to patient homes within the assigned territory; mileage reimbursement provided.
