Job Description

Deliver vital care in a rewarding role as a Home Health Registered Nurse, providing skilled nursing services to patients in the comfort of their homes on a full-time, 13-week contract. This field-based nursing opportunity focuses on ensuring quality, continuity, and compassionate, patient-centered support while collaborating closely within an interdisciplinary healthcare team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

  • Active Registered Nurse license in good standing for the state of practice
  • At least 1 year of recent home health (field) experience
  • Proficient in utilizing Home Care Home Base (HCHB)
  • Demonstrated OASIS assessment experience
  • Excellent clinical assessment and critical thinking abilities
  • Strong communication, time management, and organizational skills
  • Ability to work independently with a flexible field-based schedule
  • Valid drivers license, proof of auto insurance, and reliable transportation
  • BSN preferred
  • Background managing a 2532 point caseload in home health a plus
  • Familiarity with Medicare regulations and home health compliance standards beneficial

Core Responsibilities:

  • Conduct OASIS assessments and develop individualized patient care plans
  • Administer skilled nursing care including wound care, IV therapies, and medication management
  • Monitor patient status, documenting updates accurately and communicating promptly with physicians and team members
  • Educate patients and families regarding care protocols, medication regimens, and preventive health
  • Ensure all patient documentation in HCHB is thorough and timely
  • Collaborate with therapy, social work, and home health aide professionals for coordinated patient care
  • Participate in regular staff meetings, case conferences, and ongoing education
  • Adhere to regulatory and agency standards at all times
