Job Description

Deliver high-quality nursing care directly to patients in the comfort of their homes by joining a dedicated team as a Home Health Registered Nurse for a full-time, 13-week contract. This rewarding role calls for experienced RNs who can confidently manage patient assessments, clinical interventions, and interdisciplinary collaboration to achieve exceptional patient outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (generally 35 patient visits daily)
  • Field-based with daily travel to patient homes (mileage reimbursement provided)

Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management
  • Continuously evaluate patient progress and promptly communicate updates to physicians and team members
  • Provide thorough patient and family education concerning treatment, medications, and preventative practices
  • Document all care activities accurately and timely using Home Care Home Base (HCHB)
  • Collaborate with physical therapy, occupational therapy, medical social work, and home health aides
  • Participate in case conferences, staff development meetings, and continuing education
  • Ensure adherence to regulatory and agency standards for clinical care and documentation

Qualifications:

  • Active RN license and in good standing in the state of practice
  • At least 1 year of recent home health (field) nursing experience
  • Demonstrated proficiency with HCHB and OASIS
  • Excellent clinical assessment, communication, and organizational skills
  • Reliable transportation, drivers license, and valid insurance
  • Ability to work independently in a field setting

Preferred:

  • BSN degree
  • Experience with 2532 point caseload
  • Knowledge of Medicare and home health regulations
