A skilled and compassionate Registered Nurse is needed to provide vital home health services for patients in a field-based position. This contract role focuses on supporting individuals in their homes, delivering personalized, patient-centered care using the latest assessment tools and electronic documentation systems.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans tailored to each patient.
• Administer skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress, identify any changes in condition, and coordinate communication with the wider care team and physicians.
• Educate patients and families on managing diseases, understanding treatment plans, medication regimens, and preventive health strategies.
• Document care accurately and efficiently in the Home Care Home Base (HCHB) system.
• Collaborate closely with physical/occupational therapists, medical social workers, and home health aides to ensure optimal, coordinated care.
• Attend case conferences, team meetings, and ongoing education sessions as necessary.
• Comply with all regulatory and agency policies regarding patient care and documentation.

Requirements:

• Active RN license in good standing for the state of practice.
• At least one year of recent home health nursing (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, valid drivers license, and auto insurance.
• Excellent assessment, critical thinking, communication, and organizational skills.
• Ability to manage a flexible field-based schedule independently.

Preferred Qualifications:

• BSN degree and prior experience managing 2532 points per week.
• Familiarity with Medicare and home health compliance standards.
