Job Description

Deliver high-quality, patient-focused care as a Home Health Registered Nurse (RN) in a rewarding contract opportunity. This role involves supporting patients in their own homes, fostering independence while ensuring safe, compassionate medical attention.

Schedule and Assignment Details:

  • Monday Friday, 8:00 AM 5:00 PM
  • 13-week contract
  • Typical caseload: 2532 points per week (35 patient visits daily)
  • Field-based position with daily travel (mileage reimbursement included)
  • Documentation: Home Care Home Base (HCHB)
  • Assessment tool: OASIS

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and craft individualized care plans tailored to each patients needs.
  • Provide hands-on skilled nursing care such as wound care, IV therapy, medication administration, and monitoring of chronic conditions.
  • Assess and document patient progress, recognizing and reporting significant clinical changes.
  • Educate patients and families about diseases, treatments, preventive health, and medication regimens.
  • Accurately document care in HCHB and comply with all agency and regulatory standards.
  • Coordinate across an interdisciplinary care team, including therapists and social workers, to ensure seamless care.
  • Attend case conferences and staff meetings; participate in ongoing educational opportunities.

Qualifications:

  • Active RN license in good standing in the state of practice
  • At least 1 year of recent home health nursing experience (field work preferred)
  • Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments
  • Exceptional assessment, organizational, and communication skills
  • Reliable transportation, active drivers license, and current auto insurance
  • Ability to work independently and manage a diverse field-based caseload

Preferred:

  • Bachelors degree in Nursing (BSN)
  • Prior experience managing 2532 points per week
  • Familiarity with Medicare and home health regulatory requirements
