An exciting opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality, patient-centered care in a home health setting. This contract role centers on providing skilled nursing services directly to patients in their homes, helping to ensure excellent clinical outcomes and a positive patient experience. The ideal candidate thrives in an independent, field-based role and excels at patient communication, organization, and clinical documentation.
Schedule & Setting:
- Contract duration: 13 weeks
- Monday Friday, 8:00 AM 5:00 PM
- Field-based (patient homes)
- Caseload: 2532 points weekly (typically 35 patient visits per day)
- Daily travel required within assigned territory (mileage reimbursement provided)
Core Responsibilities:
- Complete OASIS assessments and develop individualized care plans
- Deliver skilled nursing care: wound care, IV therapy, medication administration, chronic disease management
- Monitor patient progress; identify and communicate changes to the care team and physicians
- Educate patients and their families regarding diagnoses, medications, and preventive care
- Document all care provided using Home Care Home Base (HCHB)
- Coordinate with interdisciplinary team members including therapists and social workers
- Attend case conferences, staff meetings, and continuing education as needed
- Uphold regulatory and agency standards for care delivery and documentation
Required Qualifications:
- Active RN license in good standing
- Minimum 1 year of recent Home Health RN experience (field-based experience preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Reliable transportation, valid drivers license, and auto insurance
- Strong assessment, time management, and communication skills
Preferred:
- BSN
- Previous experience managing 2532 points weekly
- Knowledge of Medicare and home health compliance
