Deliver exceptional patient care as a compassionate Home Health Registered Nurse (RN), providing skilled nursing services directly to patients in their homes. This contract opportunity offers you the autonomy to manage individualized care, support recovery and chronic illness management, and foster positive healthcare outcomesall within a field-based setting.
Schedule and Details:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract assignment
- Caseload: 2532 points weekly, typically 35 patient visits per day
- Field-based with daily travel to patient homes within the assigned area
Primary Responsibilities:
- Conduct comprehensive OASIS assessments and develop tailored care plans
- Deliver skilled nursing interventions: wound care, IV therapy, medication administration, chronic disease management
- Monitor and evaluate patient status, promptly communicating any changes to the multidisciplinary team and physicians
- Educate patients and caregivers on disease management, medications, and ongoing treatment plans
- Complete timely, accurate documentation in Home Care Home Base (HCHB)
- Collaborate with physical/occupational therapists, medical social workers, and home health aides for coordinated care
- Participate in staff meetings, case conferences, and continuing education as needed
- Maintain compliance with regulatory and agency standards
Qualifications:
- Active RN license in good standing
- Minimum 1 year of recent Home Health experience (field work preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS documentation
- Excellent clinical assessment, problem-solving, and organization skills
- Self-motivated with the ability to work independently and compliantly
- Reliable vehicle, valid drivers license, and auto insurance
- BSN and experience managing a 2532 point caseload preferred
- Familiarity with Medicare/home health compliance is a plus
