An opportunity is available for an experienced and compassionate Registered Nurse with a background in home health care. This contract role involves delivering skilled nursing services directly in patients homes, ensuring high-quality, coordinated, and patient-focused care. The position suits an RN who thrives working independently, is detail-oriented, and brings a commitment to clinical excellence and outstanding patient communication.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (generally 35 patient visits per day)
- Field-based within an assigned territory (mileage reimbursement provided)
Responsibilities:
- Conduct comprehensive OASIS assessments and develop tailored care plans
- Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease oversight
- Monitor patient progress and promptly communicate any changes to the interdisciplinary team and physicians
- Educate patients and their families about disease management, treatment plans, and prevention
- Ensure accurate, timely documentation in Home Care Home Base (HCHB)
- Collaborate with physical/occupational therapists, medical social workers, and home health aides to ensure coordinated patient care
- Participate in regular case conferences, meetings, and continuing education opportunities
- Adhere to all regulatory and agency standards for documentation and patient care
Qualifications:
- Active RN license (in good standing)
- Minimum 1 year of recent home health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong assessment, critical thinking, organization, and time management abilities
- Reliable car, valid drivers license, and auto insurance
- BSN, Medicare knowledge, and prior experience managing 2532 points weekly preferred
