Delivering high-quality patient care in a field-based setting, this Home Health Registered Nurse (RN) role focuses on providing expert clinical services to patients within their own homes. Youll independently manage a patient caseload, working Monday through Friday from 8:00 AM to 5:00 PM, and support 2532 points per week (typically 35 patient visits each day) as part of a 13-week contract assignment. Utilize your clinical skills to make a difference, maintain autonomy, and collaborate with interdisciplinary healthcare professionals.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans for each patient.
- Administer skilled nursing tasks such as wound care, IV therapy, medication management, and chronic disease monitoring.
- Monitor patient progress, recognize changes in condition, and ensure swift communication with physicians and care team members.
- Deliver thorough education to patients and families on care plans, medications, and preventative measures.
- Maintain up-to-date, accurate documentation in Home Care Home Base (HCHB).
- Work alongside therapists, medical social workers, and home health aides to coordinate and optimize patient care.
- Participate in regular case conferences and ongoing staff development meetings.
- Ensure all activities align with clinical and regulatory standards for home health.
Required Qualifications:
- Active and unencumbered RN license in the state of practice.
- At least 1 year of recent field-based Home Health experience.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
- Reliable transportation, valid drivers license, and current auto insurance.
- Ability to work independently with excellent organizational and communication abilities.
Preferred:
- BSN degree.
- Previous experience handling 2532 points weekly.
- Knowledge of Medicare and home health compliance.
