An opportunity is available for a skilled Home Health Registered Nurse (RN) to provide expert nursing care in patients homes within the Christiansburg, VA area. This full-time contract role spans 13 weeks and requires delivering compassionate, patient-centered care while operating independently in a field-based setting.

The ideal candidate will perform thorough assessments using the OASIS tool, develop personalized care plans, provide nursing interventions including wound care and IV therapy, and collaborate closely with an interdisciplinary care team. Maintaining accurate electronic documentation in Home Care Home Base (HCHB) and educating patients and caregivers on disease management are integral aspects of the position. Daily travel within the assigned territory is required, with mileage reimbursement provided.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized care plans
• Deliver skilled nursing care including medication administration, wound care, IV therapy, and chronic disease management
• Monitor patient progress and communicate changes promptly with physicians and care team
• Educate patients and families on treatment plans, medications, and preventive health
• Maintain timely and accurate documentation in HCHB system
• Collaborate with therapists, social workers, and home health aides to coordinate care
• Participate in case conferences, staff meetings, and continuing education
• Adhere to regulatory and agency standards for documentation and care

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum one year recent home health experience, preferably field-based
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments required
• Strong clinical assessment, critical thinking, and communication abilities
• Reliable transportation with valid drivers license and auto insurance
• Ability to work independently managing a flexible field-based schedule Preferred:
• BSN degree
• Experience managing 2532 patient points weekly
• Knowledge of Medicare and home health compliance standards

This contract offers a Monday-Friday schedule from 8:00 AM to 5:00 PM, managing 35 home visits per day.
