An opportunity is available for a dedicated Registered Nurse with strong home health experience to deliver high-quality care to patients in a field-based setting. This position focuses on providing skilled nursing interventions, comprehensive assessments, and ongoing support to patients in the comfort of their homes, ensuring optimal recovery and well-being.
Schedule & Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Home Care Home Base (HCHB) electronic documentation
- OASIS assessment tool
- Daily travel within assigned territory (mileage reimbursement provided)
Responsibilities:
- Complete thorough OASIS assessments and maintain individualized care plans
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management
- Continuously evaluate patient progress and communicate changes to the care team and physicians
- Educate patients and families regarding disease processes, treatment regimens, and preventive health practices
- Ensure all documentation is timely and accurate within HCHB
- Collaborate with a multidisciplinary team, participating in case conferences and professional development sessions
- Comply with regulatory, agency, and evidence-based practice standards
Required Qualifications:
- Active RN license in good standing for the state of practice
- Minimum one year of recent home health experience (field preferred)
- Proficiency in Home Care Home Base (HCHB)
- Strong OASIS experience
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent clinical assessment, organizational, and communication skills
- Self-motivated and comfortable working independently
Preferred:
- BSN degree
- Previous caseload management of 2532 points per week
- Familiarity with Medicare/home health compliance
