An opportunity is available for a skilled and dedicated Home Health Registered Nurse eager to make a meaningful impact in patients lives by providing comprehensive nursing care within their homes. This role is well-suited for a professional who is organized, compassionate, and thrives working autonomously while delivering superior clinical care and strong communication to both patients and their families.
Schedule and Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week, equaling approximately 35 patient visits daily
- Field-based, servicing patients in their residences
- Uses Home Care Home Base (HCHB) for documentation and OASIS for assessments
- Mileage reimbursement for daily local travel
Key Responsibilities:
- Perform thorough OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions including wound care, IV therapy, chronic disease management, and medication administration
- Monitor patient conditions, address changes, and communicate effectively with physicians and interdisciplinary teams
- Educate patients and loved ones on diagnoses, treatment plans, medications, and preventative health
- Maintain accurate documentation, complying with all regulatory requirements and home health standards
- Collaborate with home health aides, physical and occupational therapists, and medical social workers
- Take part in case conferences, staff meetings, and ongoing professional development
Required Qualifications:
- Current RN license in good standing (state of practice)
- At least 1 year of recent home health nursing experience
- Confidence using Home Care Home Base (HCHB) and performing OASIS assessments
- Reliable transportation, drivers license, and valid car insurance
- Excellent time management, critical thinking, and communication skills
- Ability to prioritize and work independently
- BSN and previous experience managing high-point caseloads preferred
- Knowledge of Medicare and home health compliance is a plus
