An opportunity awaits for a dedicated Home Health Registered Nurse seeking a fulfilling field-based position in an engaging contract role. This position is ideal for RNs who are passionate about delivering skilled nursing care in a patients home environment and thrive on working autonomously while upholding the highest standards of clinical assessment and care coordination.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Conduct comprehensive OASIS assessments to develop personalized nursing care plans.
• Deliver skilled nursing care including wound care, IV therapy, medication administration, and education for chronic disease management.
• Assess patient condition changes and communicate with interdisciplinary teams and physicians to ensure cohesive care.
• Guide patients and families regarding disease management, treatment protocols, medications, and preventive strategies.
• Enter thorough and timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, medical social workers, and aides for total patient care coordination.
• Attend case conferences, staff meetings, and professional development sessions as required.
• Ensure compliance with all regulatory and clinical guidelines.

Qualifications:

• Active and unrestricted RN license in this state
• At least one year of recent field-based Home Health nursing experience
• Proficiency in HCHB and demonstrated OASIS experience
• Exceptional assessment, communication, and organizational skills
• Reliable vehicle, valid drivers license, and current auto insurance
• Ability to independently manage a field schedule

Preferred:

• BSN degree
• Experience managing a 2532 point caseload
• Familiarity with Medicare and home health regulations

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
