Exciting opportunity for an experienced and compassionate Registered Nurse to provide skilled care in a home health setting. This contract role offers a rewarding environment where you can impact patient outcomes and support recovery, rehabilitation, and independence for individuals in their own homes.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient status, assess changes in condition, and ensure timely communication with care teams and physicians.
- Educate patients and families on illness management, medication, and preventive care.
- Complete thorough and timely electronic documentation in Home Care Home Base (HCHB).
- Collaborate with other healthcare professionalstherapists, medical social workers, and aidesfor coordinated patient care.
- Participate in case conferences, staff meetings, and continuing education to maintain best practices and compliance standards.
Qualifications:
- Active RN License (in good standing)
- At least 1 year of recent home health clinical experience (field experience strongly preferred)
- Proficient in Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work independently and adhere to a field-based schedule
- Excellent communication skills; BSN and Medicare/home health compliance knowledge preferred
