Take the next step in your nursing career by making a meaningful impact as a Home Health Registered Nurse. As a skilled clinician, you will deliver essential care directly to patients within their homes, supporting recovery and independence while promoting quality outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
As a Home Health RN in this 13-week contract role, youll manage a caseload of 2532 points per week (typically 35 patient visits daily). Using your expertise, youll perform OASIS assessments, implement individualized care plans, and uphold the highest standards in patient advocacy and care coordination.
Key Responsibilities:
- Conduct OASIS assessments and develop comprehensive care plans
- Deliver skilled nursing services, including wound care, IV therapy, and medication administration
- Assess patient progress, respond to changes in condition, and collaborate with physicians and care teams
- Educate patients and their families about care plans, medications, and preventive health
- Maintain timely and accurate electronic documentation in Home Care Home Base (HCHB)
- Work collaboratively with an interdisciplinary team, participating in case conferences and ongoing education opportunities
- Comply with agency and regulatory standards for home health documentation and patient care
Required Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health experience
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Solid clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and automobile insurance
- Ability to manage a flexible field-based schedule independently
- Strong organizational, time-management, and communication abilities
Preferred:
- BSN degree
- Experience managing 2532 points weekly
- Familiarity with Medicare guidelines for home health
