Job Description
An experienced and compassionate Registered Nurse is needed for a field-based home health position. This contract role focuses on delivering high-quality, patient-centered care in clients homes as part of an interdisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week engagement
- Caseload: 2532 points per week (typically 35 patient visits daily)
What Youll Do:
- Complete OASIS assessments and develop customized care plans for each patient
- Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management
- Monitor clinical progress, recognize changes in condition, and communicate promptly with the care team and physicians
- Provide education to patients and families on disease management, medications, and preventive health
- Maintain thorough, timely documentation using Home Care Home Base (HCHB)
- Collaborate closely with therapy, social work, and aide staff to ensure comprehensive care
- Participate in case conferences, staff meetings, and continuing education
Prerequisites:
- Active RN license in good standing (state of practice)
- At least one year of recent Home Health nursing experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) and demonstrated OASIS assessment experience
- Adept at clinical assessment and independent decision-making
- Reliable transportation, current drivers license, and insurance
- Strong organizational, time management, and communication abilities
Preferred:
- BSN
- Familiarity with managing a 2532 point weekly caseload
- Understanding of Medicare and home health compliance
