An opportunity is available for a skilled and compassionate Registered Nurse specializing in home health care. In this contract role, you'll deliver high-quality nursing services to patients in their homes, focusing on patient safety, comfort, and autonomy. You'll excel if you're highly organized, self-directed, and adept at working independently while fostering positive patient relationships and collaborating with interdisciplinary teams.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based/home visits, with a caseload of 2532 points per week (generally 35 patient visits daily)
- Daily territory travel; mileage reimbursement available
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum 1 year of recent home health experience (field preferred)
- Strong working knowledge of Home Care Home Base (HCHB) electronic documentation
- Proficient with OASIS assessments and electronic charting
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent clinical, communication, time management, and organizational abilities
- Ability to work independently and manage a field-based schedule
- BSN and experience with 2532 weekly points strongly preferred
Primary Responsibilities:
- Conduct thorough OASIS assessments; develop and manage individualized care plans
- Deliver comprehensive nursing care (wound care, medication administration, IV therapy, chronic disease management)
- Monitor patient progress, report changes promptly, and educate patients and families on care and prevention
- Document visits and interventions using HCHB
- Collaborate with the broader care team (therapists, aides, social workers) for coordinated patient care
- Adhere to agency and regulatory standards for care and documentation
