Job Description
A skilled and compassionate Registered Nurse with home health experience is needed for a 13-week contract supporting a patient-centered environment. This field-based role allows you to deliver high-quality nursing care directly in patients homes, ensuring uninterrupted, individualized support.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Expect a caseload averaging 2532 points per week (typically 35 patient visits per day)
Key Responsibilities:
- Complete OASIS-based assessments and craft evidence-driven care plans
- Deliver skilled nursing, including wound care, IV therapy, medication management, and chronic disease interventions
- Monitor patient conditions, recognize and promptly report changes, and coordinate with care teams and physicians
- Educate patients and families about health conditions, home care, medications, and prevention strategies
- Maintain detailed, timely documentation within Home Care Home Base (HCHB)
- Collaborate with physical therapists, occupational therapists, social workers, and aides for cohesive patient care
- Attend case conferences, team meetings, and ongoing training sessions
- Ensure compliance with all state, federal, and agency requirements
Required Qualifications:
- Active RN license in good standing (state-specific)
- Minimum one year of recent home health field nursing experience
- Demonstrated proficiency in Home Care Home Base (HCHB)
- OASIS assessment experience
- Strong clinical judgement and independent decision-making skills
- Excellent communication and organizational abilities
- Valid drivers license, reliable transportation, and auto insurance
Preferred:
- BSN degree
- Experience managing 2532 points weekly
- Familiarity with Medicare and home health compliance
