A skilled and compassionate Home Health Registered Nurse (RN) is needed to provide quality care to patients in their homes. This contract opportunity is ideal for professionals who value patient-centered care and are comfortable working independently in a field-based environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress and communicate changes promptly with the care team and physicians.
• Educate patients and families about disease processes, treatment plans, medications, and preventive care.
• Complete electronic documentation accurately and efficiently using Home Care Home Base (HCHB).
• Collaborate with an interdisciplinary team including physical therapists, occupational therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and continuing education requirements.
• Travel daily to patient homes within the assigned territory (mileage reimbursement provided).

Qualifications:

• Active RN license in good standing in the state of practice.
• At least 1 year of recent home health experience (field experience preferred).
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Strong assessment, communication, and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible field-based schedule.
• BSN preferred.
• Experience managing 2532 points per week and knowledge of Medicare home health compliance is a plus.
