In this field-based position, youll deliver skilled nursing services to patients in their homesfostering recovery, independence, and positive health outcomes. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and design personalized care plans.
• Deliver skilled interventions including wound care, IV therapy, medication administration, and disease management.
• Closely monitor patient conditions, document changes, and communicate updates promptly to physicians and multidisciplinary teams.
• Provide patient and family education regarding treatments, medications, disease management, and preventive care.
• Ensure meticulous, timely documentation using Home Care Home Base (HCHB) software.
• Coordinate care closely with therapists and other team members.
• Attend case conferences, staff meetings, and participate in continuing education when required.
• Comply with all standards governing home health care delivery and documentation.

Required Qualifications:

• Active RN license in good standing.
• At least 1 year of recent Home Health nursing experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, current drivers license, and auto insurance.
• Excellent communication, time management, and interpersonal skills.
• Ability to independently manage a flexible, field-based caseload (expectation: 2532 points per week; typically 35 patient visits daily).

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN).
• Previous experience managing similar weekly caseloads.
• Familiarity with Medicare and home health compliance.
