An experienced and compassionate Registered Nurse is sought to deliver high-quality home health care to patients in a field-based setting. This contract opportunity provides the chance to work directly with individuals in their homes, ensuring continuity, quality, and patient-centered care. The ideal nurse is self-driven, detail-oriented, and proficient in both clinical skills and patient communication, thriving in an autonomous environment while maintaining clinical excellence.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform thorough OASIS assessments and develop individualized patient care plans.
- Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, promptly report changes in condition, and communicate effectively with physicians and healthcare team members.
- Educate patients and families on care plans, medications, disease processes, and preventive health measures.
- Accurately document care using the Home Care Home Base (HCHB) system.
- Coordinate care with therapists, social workers, and aides to ensure integrated, multidisciplinary support.
- Attend case conferences, staff meetings, and participate in ongoing education.
- Maintain compliance with agency and regulatory documentation standards.
Requirements:
- Active, unencumbered RN license in the state of practice.
- At least one year of recent home health experience (field experience highly preferred).
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments.
- Demonstrated clinical assessment and critical thinking ability.
- Reliable transportation with valid drivers license and auto insurance.
- Ability to independently manage a flexible, field-based caseload.
- Strong communication, time management, and organizational skills.
Preferred:
- BSN degree.
- Familiarity in managing 2532 points per week.
- Understanding of Medicare and home health compliance standards.
