An exciting opportunity awaits for a dedicated and compassionate Registered Nurse to deliver exceptional care in a home health setting. As a Home Health RN, youll play a pivotal role supporting patients in their homes, enhancing their quality of life and fostering independence through tailored nursing interventions. This is a full-time contract position spanning 13 weeks, where your clinical expertise and patient-centered approach will make an immediate, positive impact.
Schedule:
- Full-time contract, 13 weeks
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive patient assessments using OASIS and develop individualized care plans.
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and document progress accurately in the Home Care Home Base (HCHB) system.
- Communicate promptly with physicians and care teams regarding any changes in patient status.
- Educate patients and family members on care plans, medication regimens, and preventive health strategies.
- Collaborate with interdisciplinary team members to ensure coordinated and holistic care.
- Participate in case conferences, staff meetings, and mandatory trainings.
Required Qualifications:
- Active RN license in good standing in the state of practice.
- Minimum 1 year of recent experience in home health, preferably in the field.
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Reliable transportation, valid drivers license, and current auto insurance.
- Excellent organizational, critical thinking, and communication skills.
- Strong ability to manage a flexible, field-based caseload independently.
Preferred Qualifications:
- Bachelors degree in Nursing (BSN).
- Demonstrated experience managing 2532 points per week.
- Familiarity with Medicare and home health compliance.
