An opportunity is available for an experienced and compassionate Registered Nurse (RN) specializing in home health to provide skilled nursing care to patients in their homes. This role focuses on delivering patient-centered care, maintaining clinical excellence, and working independently within a supportive team environment. The position is full-time, field-based, and offers a 13-week contract with the potential for extensions.

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate effectively with care teams and physicians.
• Educate patients and families on treatment plans, medications, disease management, and preventive care.
• Maintain accurate and timely documentation using the Home Care Home Base (HCHB) system.
• Collaborate with interdisciplinary team members such as physical therapists, occupational therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Comply with all regulatory and agency standards related to patient care and documentation.

Qualifications & Desired Experience:

• Active RN license in good standing.
• Minimum one year recent home health experience, preferably field-based.
• Proficiency in HCHB electronic documentation system.
• Experience with OASIS assessment tool.
• Strong clinical assessment, critical thinking, and organizational skills.
• Reliable transportation with valid driver's license and auto insurance.
• Ability to manage a flexible field-based schedule independently.
• BSN preferred.
• Experience managing 2532 points per week highly desirable.
• Familiarity with Medicare and home health compliance standards is a plus.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM.
• Caseload typically includes 35 patient visits daily within an assigned territory (mileage reimbursement provided).
