Job Description

We are seeking an experienced and compassionate Registered Nurse with a background in home health to provide skilled nursing care in a field-based setting. As a Home Health RN, you will play a crucial role in supporting patients health and independence within their own homes.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly identify changes in condition, and communicate with physicians and healthcare team members.
• Educate patients and families regarding disease processes, medications, and preventive care.
• Maintain timely, accurate documentation utilizing Home Care Home Base (HCHB).
• Collaborate effectively with physical therapists, occupational therapists, social workers, and home health aides for coordinated patient care.
• Participate in interdisciplinary meetings and ongoing professional development.
• Ensure compliance with all regulatory and internal documentation standards.
• Provide daily travel to patient homes within your assigned area (mileage reimbursement is provided).

Qualifications:

• Active RN license (in good standing).
• Minimum of one year of recent home health (field) experience.
• Proficiency in Home Care Home Base (HCHB).
• OASIS assessment experience.
• Reliable transportation, valid drivers license, auto insurance.
• Strong clinical skills, critical thinking, and communication abilities.
• Ability to work independently and manage a flexible caseload.
• BSN and prior experience managing 2532 points per week are preferred.
• Knowledge of Medicare and compliance in home health settings is a plus.
