Job Description

A skilled and compassionate Registered Nurse with experience in home health is needed to provide exceptional patient care in the comfort of patients homes. This full-time, 13-week contract offers the opportunity to make a meaningful impact, ensuring patients receive high-quality, individualized care and education.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Expectation of 2532 points per week (approximately 35 patient visits per day)
• Daily travel to patient homes within an assigned territory (mileage reimbursement provided)

Required Qualifications:

• Active RN license in the state of practice
• A minimum of 1 year of recent home health experience (field experience highly valued)
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Demonstrated expertise with OASIS assessments and documentation
• Reliable transportation, valid drivers license, and up-to-date auto insurance
• Strong clinical assessment, critical thinking, time management, and organizational skills
• Ability to work independently while coordinating with a diverse multidisciplinary team

Preferred Qualifications:

• BSN strongly preferred
• Experience handling 2532 points per week in home health
• Familiarity with Medicare and home health compliance standards

Key Responsibilities:

• Complete and document OASIS assessments; develop and update individualized care plans
• Deliver skilled nursing care including medication management, wound care, IV therapy, and chronic disease management
• Educate patients and families on care plans, medications, and preventive strategies
• Collaborate with PT, OT, MSW, and home health aides to ensure coordinated, holistic care
• Attend case conferences and staff meetings; participate in ongoing professional development
