An experienced Home Health Registered Nurse (RN) position is available for a dedicated professional to deliver skilled nursing care in patient homes. This contract role requires a motivated, organized nurse who excels in independent practice while maintaining clinical excellence and strong patient communication.
Responsibilities include performing comprehensive assessments using the OASIS tool, developing individualized care plans, and providing a range of skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. You will monitor patient progress, communicate condition changes promptly, and educate patients and families about treatment plans and preventive care. Accurate documentation in Home Care Home Base (HCHB) and collaboration with physical therapists, occupational therapists, social workers, and home health aides are essential. Participation in case conferences and staff meetings supports coordinated care delivery, all while adhering to agency and regulatory standards.
Key Qualifications:
- Active RN license in good standing within the state of practice
- Minimum 1 year recent Home Health nursing experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with excellent time management and organizational skills
Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 points per week caseload
- Knowledge of Medicare and home health compliance standards
Location and Schedule:
This is a full-time, 13-week contract based in Campbellsville, KY. The schedule runs Monday through Friday, 8:00 AM to 5:00 PM, with typical caseload expectations of 2532 points weekly (about 35 patient visits daily). Daily travel to patient homes within an assigned territory is required; mileage reimbursement is provided.
