As a Home Health Registered Nurse (RN), youll provide essential skilled nursing services, guiding patients and families through every step of the care process. This patient-facing, field-based role is an excellent fit for self-motivated and organized professionals who thrive on making a direct impact.

Schedule and Role Details:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract assignment
• Expect a caseload of 2532 points per week (approximately 35 patient visits daily)
• Field-based position; daily travel within the assigned territory (mileage reimbursement provided)
• Use Home Care Home Base (HCHB) for electronic documentation; OASIS assessments required

Your Key Responsibilities:

• Conduct thorough OASIS assessments and develop personalized care plans
• Deliver skilled nursing services, such as medication administration, wound care, IV therapy, and chronic disease management
• Evaluate patient progress, recognize changes in condition, and coordinate care with physicians and interdisciplinary teams
• Provide education to patients and families on disease processes, treatment regimens, and preventive care strategies
• Accurately document all activities using HCHB to ensure regulatory compliance and seamless care delivery
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides
• Participate in team meetings, case conferences, and ongoing training opportunities

Qualifications:

• Active RN license (in good standing) for the state of practice
• Minimum 1 year recent experience in Home Health (field experience strongly preferred)
• Proficient in Home Care Home Base (HCHB) and OASIS documentation
• Reliable transportation, drivers license, and auto insurance
• Strong assessment, critical thinking, and organizational skills; works well independently
• BSN and Medicare/home health compliance knowledge preferred
