Job Description
Deliver high-quality, patient-centered care as a Home Health Registered Nurse, supporting individuals in their homes to achieve optimal health outcomes. This is a full-time, 13-week contract opportunity, ideal for a self-motivated RN with a strong background in home health and a commitment to clinical excellence.
Schedule & Role Details:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, providing care in patient homes
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Daily travel within assigned territory (mileage reimbursement provided)
- Documentation system: Home Care Home Base (HCHB)
- Assessment tool: OASIS
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized patient care plans
- Deliver skilled nursing interventions (wound care, IV therapy, chronic disease education, and more)
- Monitor clinical progress, identify changes, and communicate with interdisciplinary care teams and physicians
- Educate patients and families regarding conditions, treatment, medications, and preventative health strategies
- Maintain thorough, timely documentation in HCHB
- Collaborate with therapists, social workers, and aides to coordinate holistic patient care
- Participate in case conferences, staff meetings, and ongoing education
- Uphold regulatory and agency guidelines for quality and documentation
Qualifications:
- Current, active RN license in state of practice (good standing)
- Minimum 1 year recent home health RN experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Strong clinical assessments, time management, and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently in a field-based role
- BSN preferred
- Experience managing 2532 point caseload and familiarity with Medicare/home health compliance a plus
