A rewarding opportunity awaits an experienced and compassionate Registered Nurse with a background in home health care. This contract role centers on providing high-quality, patient-centered skilled nursing services to individuals in their own homes, supporting their health and independence within the community. Candidates must excel at independent clinical work, possess strong organization skills, and demonstrate superior communicationboth with patients and as part of a broader healthcare team.
Schedule & Role Details:
- Contract position, full-time for 13 weeks
- Monday Friday, 8:00 AM 5:00 PM
- Field-based: daily travel to patient homes within an assigned territory (mileage reimbursed)
- Caseload: 2532 points per week (approximately 35 patient visits daily)
- Documentation: Home Care Home Base (HCHB)
- OASIS assessment tool utilized
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions (wound care, IV therapy, medication administration, chronic disease management)
- Monitor patient status, update care plans, and communicate changes proactively
- Educate patients and families about treatment, medication management, and preventive care
- Accurately complete documentation in Home Care Home Base (HCHB)
- Work collaboratively with an interdisciplinary team, attending case conferences and staff meetings as needed
- Adhere to regulatory and compliance standards
Qualifications:
- Active, unrestricted RN license in the state of practice
- At least 1 year of recent home health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Strong clinical assessment, planning, and case management skills
- Excellent organization, communication, and adaptability
- Reliable transportation, valid drivers license, and auto insurance
- BSN and familiarity with Medicare compliance standards are preferred
