Job Description

A skilled and compassionate Home Health Registered Nurse (RN) opportunity is available in Camden, AL, offering a 13-week full-time contract position. This role requires delivering high-quality skilled nursing care to patients in their homes, emphasizing patient-centered approaches and clinical excellence. The position involves working independently within a flexible field-based schedule, typically Monday through Friday from 8:00 AM to 5:00 PM, managing a caseload of 2532 points per week (approximately 35 patient visits daily).

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and communicate promptly with healthcare teams and physicians.
  • Educate patients and families on disease management, medications, and preventive care.
  • Maintain accurate and timely documentation using the Home Care Home Base (HCHB) system.
  • Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Follow regulatory and agency standards in documentation and care delivery.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum one year of recent home health experience with proficiency in Home Care Home Base (HCHB) and OASIS.
  • Strong clinical assessment, critical thinking, communication, and organizational skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently and manage a flexible schedule.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Experience managing a caseload of 2532 weekly points.
  • Knowledge of Medicare and home health compliance standards.
