An exceptional opportunity is available for a skilled and compassionate Registered Nurse with a strong background in home health. This essential role focuses on delivering patient-centered, skilled nursing care directly within the home environment, helping patients achieve their care goals and maintain independence.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform thorough OASIS assessments and develop individualized care plans.
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and management of chronic health conditions.
- Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and care teams.
- Educate patients and families about diagnoses, medications, treatment plans, and preventive strategies.
- Document all patient care accurately and promptly in Home Care Home Base (HCHB).
- Work collaboratively with physical therapists, occupational therapists, medical social workers, and home health aides to coordinate comprehensive care.
- Attend case conferences, staff meetings, and participate in ongoing professional development.
- Ensure strict adherence to regulatory and agency care standards.
Required Qualifications:
- Active, unencumbered RN license in the practicing state
- At least 1 year of recent home health experience, preferably in the field
- Proficiency with Home Care Home Base (HCHB) and OASIS is required
- Reliable transportation, valid drivers license, and auto insurance
- Excellent assessment, organizational, and communication skills
- Ability to work independently in a field-based schedule
Preferred Skills:
- BSN degree
- Experience managing a caseload of 2532 points per week
- Knowledge of Medicare guidelines and home health compliance
