Seeking an experienced and compassionate Registered Nurse to deliver high-quality skilled nursing care to patients in their homes. This contract position offers the chance to make a meaningful impact, ensuring patient-centered care and promoting positive outcomes while enjoying autonomy and flexibility.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Administer skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognize changes in condition promptly, and communicate with physicians and interdisciplinary care teams.
• Provide education to patients and families regarding diseases, treatments, medications, and preventive measures.
• Maintain timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care delivery.
• Attend case conferences, staff meetings, and continuing education sessions when needed.
• Uphold rigorous standards for documentation and patient care in compliance with regulatory and agency guidelines.

Qualifications:

• Active RN license in good standing within the state.
• At least 1 year of recent home health experience (field experience preferred).
• Solid proficiency with HCHB and OASIS is required.
• Strong clinical assessment and critical thinking abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Capable of independent work and excellent time-management and organizational skills.
• BSN and prior experience managing 2532 points per week preferred.
• Familiarity with Medicare and home health compliance is a plus.
