Job Description
Providing skilled nursing care in patients homes, this Home Health Registered Nurse (RN) role focuses on delivering high-quality, patient-centered care while working independently in a dynamic field-based setting. Operating within an assigned territory, you will manage 35 patient visits daily (approximately 2532 points weekly), ensuring clinical excellence and effective communication in a home care environment.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes, and communicate promptly with care teams and physicians.
- Educate patients and families on treatment plans, disease processes, medications, and preventive practices.
- Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary professionals to coordinate comprehensive care.
- Participate in case conferences, staff meetings, and continuing education.
- Comply with all regulatory and agency standards related to documentation and care delivery.
Qualifications:
- Active RN license in good standing within the state of practice.
- Minimum 1 year of recent home health nursing experience (field experience preferred).
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Strong clinical assessment, critical thinking, communication, time management, and organizational skills.
- Reliable transportation with a valid drivers license and auto insurance.
- Ability to manage a flexible, independent field-based schedule. Preferred:
- Bachelor of Science in Nursing (BSN).
- Prior experience managing 2532 points per week.
- Knowledge of Medicare and home health compliance standards.

Contract Details:
- Full-time, 13-week contract
- Schedule: Monday through Friday, 8:00 AM 5:00 PM
- Mileage reimbursement provided for daily travel
