Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care directly to patients in their homes. This role emphasizes quality, continuity, and patient-centered care in a field-based setting. The ideal candidate is organized, self-motivated, and comfortable working independently while maintaining clinical excellence and strong communication with patients.

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Provide skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate promptly with care teams and physicians.
• Educate patients and families on disease processes, treatment plans, medications, and preventive care.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and continuing education.
• Adhere to regulatory and agency standards for patient care and documentation.

Qualifications:

• Active RN license in good standing within the state of practice.
• Minimum of 1 year recent home health experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible, field-based schedule.
• Excellent communication, time management, and organizational skills.

Preferred:

• Bachelor of Science in Nursing (BSN) preferred.
• Experience managing 2532 points per week.
• Knowledge of Medicare and home health compliance standards.

Job Details:

• Location: Burlington, NC
• Contract Type: Full-time, 13-week contract
• Schedule: Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points/week (35 patient visits daily)
• Mileage reimbursement provided
