Job Description

Bring your nursing expertise and compassion to the forefront as you deliver meaningful care in a home setting. An experienced Home Health Registered Nurse is needed for a full-time, 13-week contract opportunity, focusing on promoting positive patient outcomes with clinical excellence and strong patient communication.

Schedule/Hours:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient status, promptly communicate any changes to the interdisciplinary care team and physicians.
  • Educate patients and families on their care plans, disease processes, medications, and preventative strategies.
  • Document care and all interventions accurately and efficiently in Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated care.
  • Participate in case conferences, staff meetings, and ongoing professional education.
  • Ensure all care and documentation adheres to current regulatory and agency standards.

Qualifications:

  • Active RN license (in good standing) in the state of practice.
  • At least 1 year of recent field-based Home Health experience.
  • Familiarity and proficiency with Home Care Home Base (HCHB) and OASIS.
  • Excellent clinical assessment, critical thinking, communication, and organization skills.
  • Must have reliable transportation, a valid drivers license, and current auto insurance.
  • Able to manage a flexible case schedule (2532 points per week; 35 patient visits per day).

Preferred:

  • Bachelors degree in Nursing (BSN).
  • Previous experience maintaining Home Health caseload requirements.
  • Strong understanding of Medicare and home health compliance standards.
