An exciting opportunity is available for an experienced and compassionate Registered Nurse with a background in home health care. This contract role is ideal for a skilled clinician who thrives on autonomy and finds satisfaction in delivering patient-centered care directly in patients homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

As a Home Health RN, you'll work field-based across assigned territories, delivering skilled nursing care to a caseload of 2532 points per week (typically 35 patient visits daily). Your expertise will support continuity of care, empower patients and families, and promote optimal health outcomes, all while utilizing technology and industry-recognized assessment tools.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize and communicate changes, and adjust care plans in collaboration with physicians.
• Educate patients and families about condition management, medications, and preventive strategies.
• Maintain thorough and timely documentation using Home Care Home Base (HCHB).
• Partner with physical and occupational therapists, medical social workers, and home health aides to ensure coordinated care.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Uphold agency and regulatory standards in every aspect of care and documentation.

Requirements:

• Active RN license (state of practice), in good standing.
• At least 1 year of recent home health experience, with field experience highly valued.
• Proficient in Home Care Home Base (HCHB) and OASIS assessments.
• Excellent assessment, time management, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible, field-based schedule independently.

Preferred:

• BSN degree
• Experience managing a 2532 point caseload
• Familiarity with Medicare/home health compliance

$1,845 - $1,952/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
