An experienced and compassionate Registered Nurse is needed for a home health position providing skilled care to patients in their residences. This role is well-suited for a self-motivated clinician who excels in independent practice, detailed patient assessments, and the delivery of high-caliber, patient-centered healthcare.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Expectation of 2532 points per week (typically 35 patient visits per day)
• Daily travel throughout the assigned territory (mileage reimbursement available)

Primary Responsibilities:

• Complete thorough OASIS assessments and craft individualized care plans reflecting patient needs
• Deliver skilled nursing interventions such as wound care, IV therapy, chronic disease management, and medication administration
• Monitor and document patient progress using Home Care Home Base (HCHB)
• Educate patients and families on care plans, medications, and disease processes
• Promptly identify and communicate changes in patient condition to care teams and physicians
• Collaborate with interdisciplinary colleagues including therapists, social workers, and aides to coordinate holistic care
• Participate in case conferences, team meetings, and continuing education
• Adhere strictly to regulatory and agency compliance standards

Qualifications:

• Active, unrestricted RN license in the state of practice
• Minimum 1 year of recent home health nursing experience (field experience highly preferred)
• Proficiency in Home Care Home Base (HCHB)
• OASIS assessment experience required
• Reliable transportation with a valid drivers license and auto insurance
• Strong assessment, communication, and time management skills
• BSN preferred
• Previous experience handling a 2532 point caseload a plus
• Understanding of Medicare and home health regulations desirable

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
