Job Description

An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients in their homes, ensuring personalized, high-quality care that supports positive health outcomes. This role requires a self-driven and organized professional comfortable working independently while maintaining clinical excellence and effective patient communication.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate promptly with physicians and the care team.
  • Educate patients and families on diseases, treatments, medications, and preventive care measures.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary team members such as physical therapists, occupational therapists, and social workers to coordinate care.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with all agency and regulatory standards for documentation and patient care.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • Minimum 1 year of recent Home Health experience; field experience preferred.
  • Proficiency in Home Care Home Base (HCHB) and OASIS documentation.
  • Strong clinical assessment, critical thinking, communication, and organizational skills.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to independently manage a flexible field-based schedule.
  • BSN preferred.
  • Experience managing caseloads of 2532 points per week is a plus.
  • Knowledge of Medicare and home health compliance standards preferred.

Schedule & Location:

  • Full-time, 13-week contract
  • Monday to Friday, 8:00 AM 5:00 PM
  • Field-based, visiting 35 patients daily within a defined territory (mileage reimbursement provided)
