Deliver exceptional patient care and make a meaningful difference as a Home Health Registered Nurse on a full-time 13-week contract. This role focuses on providing skilled nursing services to patients in their homes, ensuring seamless, patient-focused care with every visit. Collaborate with interdisciplinary teams while exercising autonomy and clinical judgment in the field.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments to develop and implement tailored care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient status, document changes, and maintain ongoing communication with physicians and the healthcare team.
• Educate patients and family members on disease management, medications, and healthy lifestyle practices.
• Record accurate, timely patient documentation in Home Care Home Base (HCHB).
• Collaborate effectively with therapists, social workers, and home health aides for coordinated care.
• Participate actively in case conferences, staff meetings, and continuing education.
• Adhere to agency and regulatory requirements for documentation and care delivery.
• Travel daily to patient homes within the assigned territory (mileage reimbursement provided).

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent field-based home health nursing experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical judgment, critical thinking, and communication skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• BSN and knowledge of Medicare/home health compliance standards preferred.
• Previous experience managing a caseload of 2532 points per week is a plus.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
