Experienced and compassionate Registered Nurses are invited to provide high-quality, patient-centered care in a home health setting. This full-time, 13-week contract role involves supporting patients in their transition from illness or injury to improved health, enabling them to remain safely and comfortably at home.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, promptly communicate changes to physicians and the care team, and adjust care plans as needed.
- Educate patients and their families on treatment, disease management, medications, and preventive care strategies.
- Document patient information accurately and efficiently in Home Care Home Base (HCHB) electronic system.
- Collaborate with interdisciplinary healthcare professionals including therapists and home health aides.
- Attend case conferences, staff meetings, and participate in ongoing training.
- Ensure compliance with all agency standards and regulatory requirements.
Qualifications:
- Current RN license in good standing for the state of practice.
- At least 1 year of recent home health (field-based) experience.
- Proficiency with Home Care Home Base (HCHB) and OASIS documentation.
- Strong assessment, clinical judgment, and interpersonal skills.
- Capable of managing a caseload of 2532 points per week.
- BSN and knowledge of Medicare/home health regulations preferred.
- Reliable vehicle, valid drivers license, and auto insurance required.
