A skilled nursing professional is needed to deliver high-quality care as a Home Health Registered Nurse (RN). This role focuses on supporting patients health and well-being directly in their homes. The ideal candidate combines strong clinical skills with compassion and is comfortable operating independently while efficiently coordinating care.
Schedule & Job Details:
- Assignment: Full-time, 13-week contract
- Hours: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points/week (averaging 35 patient visits per day)
- Location: Fieldwork in patients homes, with daily travel within an assigned territory
- Documentation System: Home Care Home Base (HCHB)
- Assessment Tool: OASIS
Key Responsibilities:
- Perform thorough OASIS assessments and tailor care plans for individual needs
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic condition management
- Monitor patient status, communicate significant changes, and collaborate with the care team and physicians promptly
- Provide comprehensive education to patients and families about care processes, treatment plans, and preventive strategies
- Accurately document all patient encounters and updates in HCHB
- Work closely with therapy and social work teams to maintain coordinated, patient-focused services
- Participate in case conferences and ongoing education to maintain best practices
- Ensure compliance with regulatory and agency standards
Qualifications:
- Active RN license (in good standing) for the state of practice
- At least 1 year of recent experience in Home Health (field experience preferred)
- Proficiency in HCHB and OASIS is required
- Strong assessment, communication, time management, and organizational abilities
- Access to reliable transportation with a valid drivers license and auto insurance
- BSN and Medicare compliance knowledge are pluses
