Qualifications:
- Current, unrestricted RN license
- Proven experience in providing home health nursing care
- Strong knowledge of best practices for at-home patient assessments, care planning, and documentation
- Exceptional clinical judgment and communication skills
- Ability to travel to and between patient homes as required
- Proficient use of electronic health records (EHR) systems
Key Responsibilities:
- Perform comprehensive assessments of each patients health and home environment
- Develop, implement, and evaluate individualized care plans to promote recovery and safety
- Deliver skilled nursing care such as medication administration, wound care, and health education
- Monitor patient progress and update care plans accordingly
- Serve as a liaison between patients, families, physicians, and other members of the care team
- Educate patients and families about illness management, medications, and healthy living
- Maintain accurate and timely clinical documentation following regulatory guidelines

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
