Job Description
Provide essential skilled nursing care in patients homes as a Home Health Registered Nurse (RN) delivering compassionate, patient-focused support. This dynamic role demands a self-motivated professional who excels working independently while upholding clinical excellence and clear communication with patients and interdisciplinary teams.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans tailored to patient needs
- Deliver skilled nursing serviceswound care, medication administration, IV therapy, chronic disease management
- Monitor patient progress, detect changes in condition, and communicate promptly with physicians and care teams
- Educate patients and families on treatment plans, medications, disease processes, and preventive care
- Maintain accurate and timely documentation using Home Care Home Base (HCHB) electronic system
- Collaborate with therapists, social workers, and home health aides to ensure seamless, coordinated care
- Participate in case conferences, staff meetings, and continuing education opportunities
- Adhere strictly to regulatory standards and agency policies for documentation and patient care
Qualifications & Experience:
- Active RN license in good standing in the state of practice
- Minimum one year recent home health experience with field-based nursing preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS assessment tools
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation, valid drivers license, and insurance for daily travel within assigned territory
- Ability to manage flexible field schedules independently and efficiently
- BSN preferred
- Familiarity with Medicare and home health compliance standards is a plus
- Prior caseload management of 2532 points weekly advantageous
Schedule & Work Details:
- Full-time contract, 13 weeks in duration
- Monday through Friday, 8:00 AM to 5:00 PM
- Typically 35 patient visits per day, with caseload averaging 2532 points weekly
- Field-based role with mileage reimbursement for travel to patient homes
