Embark on a rewarding opportunity as a Home Health Registered Nurse, delivering patient-centered care within the comfort of your patients homes. This full-time, 13-week contract places you at the heart of community nursing, where youll leverage your skills in clinical assessment, care planning, and health education to foster positive health outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week, approximately 35 patient visits daily

Primary Responsibilities:

• Perform comprehensive OASIS assessments to formulate individualized care plans
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient status, identify condition changes, and communicate findings to the interdisciplinary team and physicians
• Educate patients and families on illnesses, medications, treatments, and preventive strategies
• Ensure accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate closely with therapists, social workers, and aides for coordinated care
• Attend case conferences, staff meetings, and ongoing education
• Uphold regulatory and agency standards in all care and documentation practices

Required Qualifications:

• Active RN License in good standing for the state of practice
• At least 1 year of recent field-based Home Health experience
• Proficiency with Home Care Home Base (HCHB)
• Experience conducting OASIS assessments
• Reliable transportation, valid drivers license, and auto insurance
• Excellent clinical judgment, communication, and organizational skills
• Ability to manage a flexible, field-based work schedule independently

Preferred:

• BSN degree
• Experience handling 2532 points per week
• Familiarity with Medicare home health compliance standards

$1,908 - $2,019/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
