An opportunity is available for an experienced and compassionate Registered Nurse to provide vital home health care to patients. The focus of this role is delivering skilled nursing services in a home setting, ensuring patient safety, quality, and continuity of care. This is a full-time, 13-week contract with a Monday through Friday schedule from 8:00 AM to 5:00 PM. Expect to manage a caseload of 2532 points per week, typically corresponding to 35 patient visits daily. The work involves daily travel within a designated territory, with mileage reimbursement provided.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct thorough assessments using OASIS and manage individualized patient care plans.
- Deliver skilled nursing such as wound care, IV therapy, medication administration, and chronic disease management.
- Monitor and document patient progress in Home Care Home Base (HCHB).
- Educate patients and families on health conditions, treatments, and medications.
- Coordinate care with interdisciplinary care team members.
- Attend case conferences and contribute to ongoing patient care planning.
- Ensure compliance with clinical and regulatory standards.
Required Qualifications:
- Active, unrestricted RN license in the state
- Minimum of 1 year recent home health experience
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Reliable transportation, valid drivers license, and active auto insurance
- Ability to work independently and demonstrate strong assessment and organizational skills
Preferred Qualifications:
- Bachelors degree in nursing (BSN)
- Prior management of 2532 points per week
- Working knowledge of Medicare/home health compliance
