An opportunity is available for a skilled and compassionate Registered Nurse with home health expertise to provide in-home nursing care focused on patient-centered outcomes. This full-time, 13-week contract position requires delivering comprehensive nursing services to patients in their residences while maintaining a high level of clinical excellence and independent judgment.
Key responsibilities include performing detailed OASIS assessments, developing and managing individualized care plans, and administering skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. The role involves frequent communication with physicians and interdisciplinary teams, patient and family education, documentation using the Home Care Home Base (HCHB) system, and adherence to regulatory standards.
Qualifications and Desired Experience:
- Active and valid RN license in the state of practice
- At least 1 year of recent home health nursing experience, field experience preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with a flexible schedule
- BSN preferred
- Experience managing a caseload of 2532 points weekly
- Knowledge of Medicare and home health compliance standards is a plus
Job Details:
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- Caseload: Typically 35 patient visits per day, equating to 2532 points
- Setting: Field-based with travel to patient homes (mileage reimbursement provided)
- Documentation: Home Care Home Base (HCHB) electronic system
- Contract type: 13-week full-time contract with potential extensions
