This role requires compassionate, self-driven professionals comfortable with independent fieldwork, comprehensive assessments, and collaborative care planningall focused on supporting patients in their own homes.

Schedule & Details:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract assignment
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based position with daily travel (mileage reimbursement provided)
• Electronic documentation via Home Care Home Base (HCHB)
• OASIS assessment tool utilized

Key Responsibilities:

• Complete OASIS assessments; develop tailored care plans based on patient needs and physician directives.
• Deliver direct skilled nursing care: wound care, medication administration, IV therapy, chronic disease management, and health education.
• Monitor clinical progress, flag and communicate changes in patient status, and adjust care accordingly.
• Document all patient interactions in HCHB accurately and efficiently.
• Coordinate and communicate effectively with multidisciplinary team members, including therapists, social workers, and home health aides.
• Participate in case conferences and staff meetings; ensure compliance with regulatory and agency care standards.

Qualifications:

• Valid, active RN license (state of practice)
• Minimum 1 year recent home health experience (field preferred)
• Proficient in HCHB and experienced with OASIS
• Strong assessment, organizational, communication, and time-management skills
• Ability to work independently with excellent critical thinking
• Reliable transportation, valid drivers license, and proof of auto insurance

Preferred:

• BSN degree
• Previous experience with caseload management at 2532 points per week
• Familiarity with Medicare and home health compliance standards

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
