Job Description

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes, ensuring high-quality, patient-centered outcomes. In this field-based role, you will leverage your clinical skills, autonomy, and organization to facilitate optimal patient experiences and care plan compliance throughout a 13-week contract.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions, including wound care, medication management, IV therapy, and chronic disease monitoring.
• Monitor patient progress; identify and promptly report any changes in condition to the multidisciplinary team and physicians.
• Provide education to patients and families regarding diagnosis, medications, and at-home care.
• Maintain timely, accurate electronic documentation through Home Care Home Base (HCHB).
• Coordinate and collaborate with therapists, social workers, and home health aides to provide holistic care.
• Participate in regular case conferences and required continuing education.
• Ensure compliance with regulatory and agency standards for all clinical documentation and care.

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent home health nursing experience (field-based highly preferred).
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Robust clinical assessment, critical thinking, and organizational skills.
• Reliable vehicle, valid drivers license, and current auto insurance.
• Ability to thrive independently and manage complex schedules.
• Strong interpersonal and communication abilities.

Preferred:

• BSN degree.
• Experience managing a caseload of 2532 points per week.
• Familiarity with Medicare and home health compliance.
