An experienced and compassionate Registered Nurse is needed to deliver high-quality, patient-centered care in a home health setting. This full-time, 13-week contract role offers the chance to work autonomously in the field and build meaningful relationships with both patients and their families. The ideal candidate will be motivated, organized, and skilled in conducting comprehensive assessments while ensuring clinical excellence.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care (e.g., wound care, medication administration, IV therapy, chronic disease management).
• Monitor patient progress, communicate changes to the care team and physicians promptly.
• Educate patients and families about conditions, care plans, and preventive health.
• Document care accurately and efficiently using Home Care Home Base (HCHB).
• Collaborate effectively with physical therapists, occupational therapists, social workers, and aides.
• Participate in staff meetings, case conferences, and continuing education opportunities.
• Uphold all regulatory and agency documentation and care standards.

Qualifications:

• Active RN license (in good standing) in SC.
• At least 1 year of recent home health nursing experience in the field.
• Proficient in Home Care Home Base (HCHB) and OASIS.
• Strong clinical assessment, organizational, and communication skills.
• Reliable transportation, valid drivers license, and automobile insurance required.
• Ability to manage a flexible caseload independently (2532 points/week).
• BSN and prior Medicare/home health compliance knowledge preferred.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
