Exciting opportunity for a dedicated Registered Nurse with a passion for home health care! This full-time, 13-week contract position focuses on delivering skilled nursing services to patients in their homes. This role is ideal for an organized RN comfortable working independently, driven by clinical excellence, and eager to make a meaningful impact in patients lives.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, providing care directly in patient homes
• Caseload of 2532 points per week (typically 35 patient visits per day)
• Daily travel required within the assigned territory, with mileage reimbursement

Core Responsibilities:

• Deliver comprehensive OASIS assessments and design individualized care plans
• Provide skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Closely monitor patient progress, communicate changes to care teams and physicians
• Educate patients and families about diagnoses, treatment plans, and medication management
• Ensure accurate documentation in Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams: physical and occupational therapists, social workers, and aides
• Participate in meetings, case conferences, and pursue continuing education
• Observe all regulatory standards regarding patient care and documentation

Required Qualifications:

• Active RN license in good standing
• At least 1 year of recent home health experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS
• Excellent clinical assessment, organization, and communication skills
• Valid drivers license, reliable vehicle, and current auto insurance
• Ability to work independently with strong time management

Preferred Attributes:

• BSN degree
• Previous experience managing 2532 points per week
• Familiarity with Medicare and home health compliance
