Job Description
An experienced and compassionate Registered Nurse (RN) with a home health specialty is needed to provide skilled nursing care directly in patient homes. This role supports quality, continuity, and personalized care by performing thorough assessments, developing tailored care plans, and coordinating with interdisciplinary teams. The ideal candidate is motivated, organized, and thrives working independently while upholding clinical excellence and patient communication.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and create individualized care plans.
- Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, detect condition changes, and communicate updates to physicians and care teams.
- Educate patients and families on disease management, treatment plans, medications, and preventive measures.
- Maintain accurate, timely documentation within Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and aides to ensure seamless patient care.
- Participate in case conferences, staff meetings, and continuing education.
- Adhere to regulatory and agency standards for patient care and documentation.
Qualifications:
- Active RN license in good standing within the state of practice.
- Minimum of 1 year recent home health experience; field experience preferred.
- Proficient with Home Care Home Base (HCHB) and OASIS assessments.
- Strong clinical assessment, critical thinking, and communication skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently with flexible scheduling.
Preferred:
- BSN degree.
- Experience managing 2532 patient care points weekly.
- Familiarity with Medicare and home health compliance standards.
This full-time, 13-week contract position offers a Monday-Friday, 8:00 AM to 5:00 PM schedule, typically managing 35 patient visits daily with mileage reimbursement provided for travel within the assigned territory.
